Registered Nurse - Residential

Following the Four Quadrant Clinical Integration Model of the National Council's Primary Care-Mental Health Collaborative Care Project, the Nurse, under the supervision of the Nurse Team Lead incorporates the skills of a nurse, care manager, educator, and patient advocate. The Nurse provides liaison coordination between the client, Behavioral Health Providers, Primary Care Providers, and peer supports, based on client need and determination of services. This nurse also provides counseling, case management, and education to clients as related to their medical and behavioral disease states while helping to ensure clients are engaged to health and wellness services. This nurse also provides injections, routine physical care and infusions as indicated by provider orders.

EDUCATION AND EXPERIENCE

Associates Degree in Nursing from an Accredited School of Nursing, and current, unencumbered Arizona License. Experience in the use of evidence-based care, managing complexity, active engagement of clients/ families/caregivers. The Nurse should understand interdisciplinary-team care, theories/strategies for individualized care and behavioral change, quality improvement, and delivery of services across an episode of acute care. At least one year experience in primary care and/or one year experience in Behavioral Health preferred.
