The Direct Support Provider, under professional supervision, accepts life skills educational assignments from Supervisor or other designated personnel, participates in client staffing’s, develops and implements living skills training assignments according to frequency and intensity determined on the current treatment plan, coordinates these assignments with case management activities, maintains client progress notes for interventions assigned to them, and performs other duties as assigned.

The Medical Assistant/PCT and BHPP, also know as an Anywhere Care Specialist, is a key part of the success of our integrated healthcare community-based services. This individual will provide mobile physical and behavioral health care in the community. The Medical Assistant/PCT & BHPP role includes facilitating physical health, behavioral health care in the home, community and office utilizing virtual medical and behavioral health supports. The Medical Assistant/PCT and BHPP provides care management activities and supportive counseling to clients. This position is responsible for providing rapid access to care for requesting patients.

The Medical Assistant/Patient Care Technician & BHPP is a full-time or part-time, non-exempt, hourly position working Monday through Friday business hours (some flexibility may be offered) and working remotely in the greater Phoenix community. This position requires significant travel in the Greater Phoenix Area and possibly some Statewide travel.

The Peer Support Specialist, as part of the crisis mobile team, provides recovery-oriented training, outreach, and support services to individuals and groups within the adult SMI population. Encourages clients to pursue recovery-oriented activities both within the agency and the community by modeling a positive frame of mind, hope and self-determination. Assists in advocacy of client needs and rights. Provides living skills training, participates in client consultations. Assists in development and implementation of Behavioral Health Service Plans, coordinates these assignments with crisis response activities. This opening is for 30-35 hours per week on the crisis mobile team.

The Care Manager Lead supervises SMI Case Management staff including coordinates treatment activity, participates in client staffing, implements treatment plans, provides rehabilitation services, provides general health care management activities, and maintains client records. CM Lead will have direct reports. This position is partially remote, partially in office based out of Phoenix, Prescott, and Cottonwood.

The Residential Recovery Coach (RRC) assists residents with substance use and mental health disorders, under the direction of clinical staff. This includes assisting residents with daily schedules, leading residents in educational and recreational activities, and accompanying residents to and from activities and appointments. Supports the delivery of individual resident care at Momentum with significant interaction with residents in their efforts toward recovery. Provides day-to-day delivery of services (cleaning, meal preparation, monitoring environment for safety); participates in staff/residents groups and meetings; monitors and documents resident activities.

The High Needs Care Manager, Crisis, under professional supervision, coordinates treatment activity, participates in client staffing, implements treatment plans, provides rehabilitation services, provides general health care management activities, and maintains client records. Crisis Response High Needs Care Managers are primarily responsible for providing a high level of crisis assessments and intervention in community settings. Must be able to travel county wide and provide in home services to high risk youth within the community.

The Case Manager - Children's Behavioral Health coordinates treatment activity, participates in client staffing, implements treatment plans, provides rehabilitation services, provides general healthcare management activities, and maintains client records. As healthcare reform advances and health homes become the norm, the Case Manager - Children's Behavioral Health is required to expand their capacity in healthcare navigation, build on existing skills in behavior health change, and gain a better understanding of the common health problems and basic interventions for individuals with mental illness and those with chronic health problems.
Case Manager - Children's Behavioral Health interacts with clients and staff in a culturally competent manner that takes into account factors such as age, developmental or acquired disabilities, religion and spiritual orientation, ethnicity, socioeconomic status, sexual orientation, indigenous heritage, national origin, gender, and gender identity. Enhances professional growth and keeps informed of evidence-based practices in behavioral health field related to populations served and modalities used to include working knowledge of DSM V, and its criteria for mental health disorders. Follows policies and procedures and performs other duties as assigned by supervisor.

The Crisis Response Specialist, under professional supervision, accepts direct calls from Community Response Network (CRN), law enforcement, first responders and supervisors for prompt community response on a flexible schedule that may include providing client transport to medically necessary services and well checks.

Crisis Response Specialist is able to perform crisis assessments in the community and ED, has strong verbal and written clinical skills, is able to staff cases for inpatient and safe plans with BHMP's and supervisors. Able to work closely with law enforcement, first responders, persons receiving crisis services and their supports under stressful situations at all hours in a professional and calm manor. Crisis Response Specialist is able to write Title 36 applications, has broad knowledge of A.R.S., area resources, funding streams and insurances to either place clients in community with supports, in network or other hospitals according to the level of care needed.

Opening for a full time, overnight shift position in Phoenix. As a community-based position, significant travel will be required and we provide company vehicles and/or mileage reimbursement for this purpose.
