Position Summary

The population health specialist (PHS) is responsible for helping Spectrum to meet performance improvement targets related to hospital and emergency department utilization as well as other population health and programmatic goals. Facilitating interdepartmental process improvement projects and root cause analysis meetings. Effectively manage care-gap closures through partnering with health plans and other external partners. Monitoring, reporting, and responding to health plan queries, quality of care concerns, and corrective action plans. Evaluating outcomes for population health and generating reports of findings. Effectively communicating and participating with health plans and other stakeholders in population health initiatives. Engaging appropriate Spectrum Healthcare Group staff in health plan initiatives and performance improvement projects.Extracting and analyzing data from electronic medical records and other sources. Presenting program results to both internal and external audiences.

Required education and experience:

A minimum of 6 years combined higher education (Master's in Public Health preferred) and experience in primary care

or behavioral health service delivery, practice transformation, healthcare administration, health data analytics, claims

management, population health, and/or quality management
