reform advances and health homes become the norm, behavioral health Care Managers are required to expand their capacity in
healthcare navigation, build on existing skills in behavior health change, and gain a better understanding of the common health
problems and basic interventions for individuals with mental illness and those with chronic health problems.
EDUCATION AND EXPERIENCE
Care Managers must be a Behavioral Health Technician. At Spectrum, a BHT has: Minimum of a high school diploma and at least 2
years of behavioral health experience (may be combined educational and work experience). OR, An individual with a bachelor’s degree
(any) and at least 1 year experience in behavioral health. OR, An individual with a bachelor’s degree in a human services related field.
OR, An individual who completes a behavioral Health Technician training program through an educational institution. OR, An individual
with a master’s degree in a human services related field and is not licensed.
One year of Behavioral Health experience preferred.
For High Needs Care Managers: two years as a care manager and a bachelor’s degree in a human services-related field, or an
equivalent combination of experience and education preferred. Required training for position if not completed at time of hire, must be
completed within 6 months of selection.
POSITION ACCOUNTABILITIES

For High Needs Care Managers
19. Provides High Needs Case Management services to adults and children enrolled in the behavioral-health system who meet the
criteria for high needs including Seriously Mental Ill (SMI).
20. Provides weekly contact with assigned clients, which includes home/community visits, attending medication checks, etc.
21. Assists adults and youth in navigating admission and then discharge from hospitals, jails or any other out of home treatment
placement, in finding intensive services as clinically indicated, and working to decrease behaviors to improve stability in the
home & community.
22. Coordinates services and collaborates with other involved agencies, such as Department of Child Safety (DCS), Probation/
Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
23. Coordinates and facilitates Child & Family Team (CFT) and Adult Recovery Team(ART) meetings.
24. Completes referrals, prior authorizations, and other paperwork required by the health plans to enroll clients in services.
25. Collaborates with clinical team to ensure that Integrated Service Plans, CFT/ART Meeting notes, and all required paperwork is
completed accurately and in a timely manner.
26. Works with clinical team to transfer clients to the appropriate level of case management for that client.

JOB SKILLS AND ABILITIES
• Provides clinically appropriate care management activities including coordination of behavioral and physical
healthcare with Primary Care and Psychiatric Providers, both internal and external.
• Basic understanding of common health problems and interventions (i.e. obesity, smoking, nutrition, health
promotion/wellness) and demonstration of skills in behavior health change.

BASIC JOB REQUIREMENTS
