Spectrum Healthcare is seeking a compassionate and driven Hospital & Housing Specialist to join our growing team. This unique role combines hospital discharge coordination and housing support services, helping individuals successfully transition from inpatient care into stable housing and community-based services.

What You'll Do

Hospital Discharge Coordination

  • Partner with hospitals, behavioral health facilities, physicians, nurses, social workers, and case managers to coordinate safe and timely discharges.
  • Develop individualized transition plans that connect members to ongoing behavioral health, medical, and community resources.
  • Complete assessments, treatment plans, care coordination documentation, and required member records.
  • Advocate for members and help remove barriers to successful community reintegration.
  • Coordinate outpatient services, medication management, crisis services, peer support, and follow-up care.

Housing Support Services

  • Utilize a Housing First approach to help members secure and maintain stable housing.
  • Assist with housing searches, applications, lease preparation, and housing stability planning.
  • Submit and manage AHCCCS Housing Program (AHP) applications.
  • Collaborate with landlords, housing agencies, and health plans to support housing placements.
  • Connect members to community resources, housing subsidies, eviction prevention programs, and vocational services.
  • Support members throughout the housing process while promoting independence and long-term success.

What We're Looking For

Candidates must qualify as a Behavioral Health Technician (BHT) under Arizona regulations and meet one of the following:

  • High school diploma/GED and at least 2 years of behavioral health experience.
  • Bachelor's degree and at least 1 year of behavioral health experience.
  • Bachelor's degree in a human services-related field.
  • Completion of an accredited Behavioral Health Technician training program.
  • Master's degree in a human services-related field (non-licensed).

Preferred Qualifications

  • Experience in behavioral health, case management, hospital discharge planning, or housing services.
  • Knowledge of AHCCCS Housing Programs, Permanent Supportive Housing (PSH), and Housing First principles.
  • SOAR and/or VI-SPDAT certification preferred.
  • Strong communication, problem-solving, and relationship-building skills.

Requirements

  • Must be at least 21 years old.
  • Valid Arizona Driver's License with a clean driving record.
  • Ability to obtain a Level 1 Fingerprint Clearance Card.
  • Current CPR/First Aid certification (or ability to obtain).
  • Ability to travel throughout Arizona as needed.
