Children's High Needs Case Manage

As a Children's High Needs Case Manager, you will be an important part of each client's care team. You will help coordinate services, support treatment goals, connect clients with community resources, and encourage healthier choices. What You'll Do

• Coordinate behavioral health treatment and care management services.
• Participate in client staffing and treatment-planning meetings.
• Complete initial assessments and reassessments under appropriate supervision.
• Develop, implement, and update individualized treatment plans.
• Provide clinically appropriate supportive counseling.
• Help clients work toward their treatment and wellness goals.
• Connect clients with healthcare providers, community programs, state agencies, and other helpful resources.
• Assist clients with navigating behavioral healthcare and general healthcare services.
• Coordinate care with physicians, clinical staff, case management coordinators, and community partners.
• Maintain timely, accurate, and easy-to-understand client records.
• Document services and interventions using the DIAP format.
• Clearly connect documented services and client progress to treatment-plan goals.
• Follow all federal, state, RBHA, licensing, accreditation, and payer requirements.
• Maintain confidentiality and follow agency policies, safety standards, infection-control practices, and productivity expectations.
• Provide excellent customer service to clients, families, coworkers, and community partners.
• Deliver culturally responsive services that respect each client's identity, background, abilities, beliefs, and lived experiences.

Minimum Qualifications

Candidates must meet at least one of the following Behavioral Health Technician qualifications:

• High school diploma or equivalent with at least two years of behavioral health experience. A combination of relevant education and work experience may be considered.
• Bachelor's degree in any field with at least one year of behavioral health experience.
• Bachelor's degree in a human services-related field.
• Completion of a Behavioral Health Technician training program through an educational institution.
• Master's degree in a human services-related field without professional licensure.

Preferred Qualifications

High Needs Care Manager position, the following is preferred:

• At least two years of care management experience; and
• A bachelor's degree in a human services-related field; or
• An equivalent combination of relevant education and experience.

Any position-specific training that has not been completed at the time of hire must be completed within six months of selection.
