Spectrum Healthcare is looking for a compassionate and motivated Case Manager to help individuals overcome barriers, navigate behavioral healthcare, and work toward healthier, more independent lives.

What You'll Do

No two days will look exactly the same! You'll work directly with clients, providers, and community partners to coordinate care and keep treatment goals moving forward.

  • Coordinate care and treatment services for assigned clients.
  • Help clients access healthcare, benefits, and community resources.
  • Participate in client staffing and put treatment plans into action.
  • Complete assessments and reassessments under professional supervision.
  • Develop and update individualized treatment plans.
  • Provide supportive counseling and document services using the DIAP format.
  • Collaborate with healthcare providers, community agencies, and internal teams.
  • Advocate for clients and help them navigate complicated healthcare systems.
  • Maintain accurate and timely client records.
  • Participate in team meetings, training, and other activities.
  • Maintain clinical productivity and compliance standards.
  • Bring professionalism, cultural awareness, and a team-first attitude to your work.

You May Be a Great Fit If You Have

Candidates must meet at least one of the following qualifications:

  • High school diploma or GED with two years of behavioral health experience.
  • Bachelor's degree in any field with one year of behavioral health experience.
  • Bachelor's degree in a human services-related field.
  • Completion of a Behavioral Health Technician training program.
  • Non-licensed master's degree in a human services-related field.

Bonus Points

  • At least one year of behavioral health experience.
  • For High Needs Case Manager positions, two years of case management experience and a relevant bachelor's degree, or an equivalent combination of education and experience.

A Few Important Requirements

  • Must be at least 18 years old.
  • Valid driver's license and an acceptable driving record.
  • Ability to obtain a Level 1 Fingerprint Clearance Card.
  • Current CPR and First Aid certification, or the ability to obtain certification within six months of hire.
  • Basic computer skills and comfort using electronic documentation systems.
  • Ability to work flexible hours as needed.
  • Ability to occasionally lift up to 15 pounds.
