Make a Difference in the Lives of Adults Living with Serious Mental Illness (SMI)

Spectrum Healthcare is seeking a compassionate, motivated, and community-focused SMI Care Manager to join our team in Cottonwood, Arizona. In this role, you will work directly with adults diagnosed with Serious Mental Illness (SMI), helping them access behavioral health services, navigate complex systems of care, maintain stability in the community, and achieve their recovery goals.

As an SMI Care Manager, you will serve as a critical link between clients, providers, families, hospitals, community agencies, and support systems. You will advocate for individuals with complex behavioral health needs while promoting recovery, independence, wellness, and long-term success. This position is based on Spectrum's Care Manager and High Needs Care Manager responsibilities.

What You'll Do

• Provide care management services to adults enrolled in the behavioral health system who meet SMI criteria.
• Maintain regular contact with assigned members through community visits, home visits, office visits, and other approved service locations.
• Coordinate behavioral health, physical health, housing, employment, benefits, and community support services.
• Assist individuals transitioning into and out of hospitals, residential treatment settings, jails, and other levels of care.
• Facilitate Adult Recovery Team (ART) meetings and support integrated treatment planning.
• Collaborate with psychiatrists, therapists, primary care providers, family members, and community partners.
• Complete referrals, prior authorizations, and service coordination activities.
• Conduct assessments and reassessments under supervision.
• Develop and update treatment plans in accordance with agency and payer requirements.
• Provide supportive counseling and rehabilitation services while documenting services using the DIAP format.
• Support crisis intervention, stabilization efforts, and recovery-oriented care planning.
• Maintain accurate and timely clinical documentation.
• Ensure compliance with all federal, state, RBHA, and payer requirements.

What We're Looking For

Minimum Qualifications

Candidates must qualify as a Behavioral Health Technician (BHT) and meet one of the following:

• High school diploma or GED and at least two (2) years of behavioral health experience.
• Bachelor's degree in any field and at least one (1) year of behavioral health experience.
• Bachelor's degree in a human services-related field.
• Completion of a Behavioral Health Technician training program.
• Master's degree in a human services-related field (non-licensed).

Preferred Qualifications

• One (1) year or more of behavioral health experience.
• Experience working with adults diagnosed with Serious Mental Illness (SMI).
• Experience coordinating care with hospitals, probation, housing providers, and community agencies.
• For High Needs Care Management, two (2) years of care management experience and a bachelor's degree in a human services-related field, or an equivalent combination of education and experience.

• Knowledge of community resources and healthcare navigation.

• Working knowledge of DSM-5 diagnostic criteria.
• Strong documentation, communication, and organizational skills.
• Ability to build rapport with diverse populations.
• Commitment to person-centered, trauma-informed, and culturally responsive care.

Requirements

• Must be at least 18 years of age.
• Valid Arizona driver's license with an acceptable driving record.
• Ability to obtain and maintain a Level 1 Fingerprint Clearance Card.
• Current CPR/First Aid certification or ability to obtain certification within six months of hire.
• Basic computer skills and experience utilizing electronic health records.
• Ability to work flexible hours as needed.
