What You'll Do

As a Care Manager, you'll play a key role in supporting clients and coordinating their care.

• Coordinate care and support treatment planning
• Provide counseling and rehabilitation services
• Conduct assessments and update care plans
• Help clients navigate healthcare and community resources
• Collaborate with providers, agencies, and care teams
• Maintain accurate and timely documentation
• Support clients with real-life challenges and health goals

What You Bring

We're looking for compassionate, driven individuals who want to grow in behavioral health.

You qualify if you meet one of the following:

• High school diploma + 2 years of behavioral health experience
• Bachelor's degree + 1 year of behavioral health experience
• Bachelor's degree in a human services field
• Completed Behavioral Health Technician (BHT) training program
• Master's degree in a human services field (not licensed)

Preferred:

• 1+ year of behavioral health experience

Requirements

• Must be 18+
• Valid driver's license with clean record
• Ability to obtain Level 1 Fingerprint Clearance Card
• CPR/First Aid certification (or willingness to obtain)
• Basic computer and documentation skills
