Care Manager (Behavioral Health)

Role Overview:
Coordinates client care under supervision by implementing treatment plans, providing rehabilitation and support services, managing health needs, and maintaining accurate client records. As healthcare evolves, Care Managers expand skills in healthcare navigation and managing common physical and mental health conditions.

Education & Experience

  • Must qualify as a Behavioral Health Technician (BHT), which includes one of the following:
    • High school diploma + 2 years behavioral health experience
    • Bachelor's degree + 1 year behavioral health experience
    • Bachelor's in human services field
    • Completion of BHT training program
    • Master's in human services field (not licensed)
  • 1+ year experience preferred
  • High Needs Care Managers: 2+ years experience + relevant bachelor's preferred

Key Responsibilities

  • Follow policies, confidentiality, and compliance standards
  • Provide supportive counseling and maintain accurate documentation (DIAP format)
  • Assist with assessments and treatment planning under supervision
  • Coordinate care with providers, agencies, and community resources
  • Maintain productivity standards and participate in team meetings
  • Deliver culturally competent care
  • Stay current with behavioral health practices and DSM-5 knowledge
  • Support clients in navigating healthcare systems

For High Needs Clients:

  • Provide intensive case management (including SMI populations)
  • Conduct regular client contact (home/community visits)
  • Assist with transitions (hospital, jail, placements)
  • Coordinate with agencies (schools, probation, DCS, etc.)
  • Facilitate team meetings (CFT/ART) and complete required paperwork

Skills & Abilities

  • Care coordination across behavioral and physical health
  • Knowledge of common health conditions (e.g., obesity, smoking)
  • Crisis assessment and intervention skills
  • Understanding of DSM-5 diagnostic criteria
  • Strong communication and organizational skills

Basic Requirements

  • Age 18+
  • CPR/First Aid certification (unless fully remote)
  • Ability to multitask, work under stress, and use basic computer systems
  • Valid driver's license (if required)
