Working as a team member at Momentum's Women's or Men's division, Spectrum's Residential Center in Cottonwood, Arizona, the DSP participates in client staffings, develops and implements daily living skills training, coordinates these assignments with case management activities, maintains client progress notes for interventions assigned to them, and performs other duties as assigned. Momentum is a 24-hour residential treatment center; some positions will require nighttime/overnight and/or weekend hours.
The Clinical Manager assists in planning and managing clinical programs and services, and is responsible for supervision of staff and monitoring of key performance indicators. The Clinical Manager is responsible for the submission of required program reports and program compliance for the assigned department(s)/team(s)/program(s).
The Clinical Manager, under general supervision, may provide individual, parent/child, and couples assessments that lead, through diagnosis, to the client’s assignment to treatment in the child, chemical dependency, SMI, and adult general mental health service areas. In addition, the Clinical Manager may provide immediate crisis intervention, participate in client staffings, and develop portions of the client Treatment Plan in conjunction with the intake services as needed. The Clinical Manager will assist in providing initial client care, coordination through client staffings leading to service assignment, maintain client records regarding the intake, and perform other related duties. Other assigned duties may include: Coordination of intakes; communicating changes/updates in policy and procedure with the assigned team; leading the daily staffing or delegating; advising team on most clinically appropriate decisions during daily operations; and keeping the communication and coordination of workflow in order to facilitate customer service both internally and externally.
The Clinical Manager position is a full-time, exempt, salary position working Monday through Friday business hours in the Greater Phoenix area at our Cencopa Office, 11024 N. 28th Drive, Phoenix, AZ. Some in-state travel is anticipated for this position as needed.

Position Summary
The Care Manager on our Kid’s Team, under professional supervision, coordinates treatment activity, participates in client staffing, implements treatment plans, provides rehabilitation services, provides general health care management activities, and maintains client records. As healthcare reform advances and health homes become the norm, behavioral health Care Managers are required to expand their capacity in healthcare navigation, build on existing skills in behavior health change, and gain a better understanding of the common health problems and basic interventions for individuals with mental illness and those with chronic health problems. Will work with different therapists to coordinate client care.
Required Qualifications
· High School Diploma and Minimum 2 years’ experience in behavioral health; OR
· Bachelor’s degree (any) and at least 1 year experience in behavioral health; OR
· Bachelor’s degree in a human services related field; OR
· Master’s degree in a human services related field and is not licensed
· Excellent customer service skills
· Ability to interact with clients and staff in a culturally competent manner
· Ability to develop and implement treatment plans and treatment plan update in compliance with documentation guidelines
· Ability to document and present treatment interventions and care management so that progress and diagnostic issues are all clear, understandable, and relate to treatment plan goals
· Maintains clinical productivity as defined in the Spectrum Clinical Work Standards
· Ability to obtain a Level I Fingerprint Clearance Card.
· Maintains current CPR/First Aid certification
· Maintains an Arizona driver’s license and clean driving record
Preferred Qualifications
· Basic understanding of common health problems and interventions.
· Ability to demonstrate skills in behavior health change.
· Other duties as assigned or required.

The Behavioral Health Therapist performs a wide variety of clinical care activities. The Behavioral Health Therapist conducts diagnostic evaluations to assess and assign clients to appropriate levels of care and conducts individual, family, and group services, as needed. Develops and implements treatment plans, provides case management services, and performs coordination of clinical care activities.
The Behavioral Health Therapist provides clinically appropriate risk assessment and crisis intervention services. Assigned with responsibility to maintain high utilization of time in direct service (DS). Participates in performance improvement activities.
The Behavioral Health Therapist position is a full-time, exempt, salaried position working Monday through Friday business hours (some flexibility may be offered). Opportunities are fully in person either in office in or in the community.
The Family Nurse Practitioner, under Direct Supervision of the Chief Medical Officer, supports and advances the Spectrum Healthcare Group mission by providing a wide range of primary health care services which encompasses health promotion, injury and illness prevention, diagnosis, treatment and monitoring of commonly encountered health problems. Along with performs testing, referral to physicians as needed and writes prescriptions. The Family Nurse Practitioner participates in quality improvement activities and reviews, as requested. Performs other duties as assigned.
The Direct Service Provider (DSP) coordinates and collaborates with primary therapists, families and other agencies to work with particular at-risk children. The DSP provides and documents Living Skills, Family Support, Case Management, and Transportation services as described on the child's Behavioral Health Service plan.
