The Direct Service Provider coordinates and collaborates with primary therapists, families and other agencies to work with particular at-risk children. This position provides and documents Living Skills, Family Support, Case Management, and Transportation services as described on the child's Behavioral Health Service plan.
High School diploma or equivalent.
Minimum two years experience working with children and their families.
Prefer experience working with children with mental health issues.
