The Care Manager coordinates treatment activity, participates in client staffing, implements treatment plans, provides rehabilitation services, provides general health care management activities, and maintains client records. Conducts and documents family and group services and meetings,especially those geared towards substance abuse recovery. May assist with drug court liaison work. Provides clinically appropriate risk assessment and crisis intervention services. Assigned with responsibility to maintain high utilization of time in direct service (DS).
I. Bachelor's degree in any field and two years experience working with population (SMI or children, as appropriate) OR High school diploma or GED plus any combination of behavioral health education, training and experience totaling four years;
II: Bachelor's degree in a behavioral health field and 1 years behavioral health work experience.
III: Master's Degree in a behavioral health field OR Master's degree in any field with 30 semester hours of behavioral health courses and 1 years of behavioral health work experience.
